LUMED Privacy Notice

La Union Medical Diagnostic Center & Hospital, Inc. (LUMED) is committed to deliver good quality healthcare services and in line with our vision and mission, we are compelled to implement safeguards to protect your privacy and keep your personal and sensitive data safe, secure, with integrity and in accordance with the requirements of the Data Privacy Act of 2012 and its implementing rules and regulations.

This is not a consent form but a notification on how LUMED processes Personal Data and Sensitive Personal Information of our Data Subjects. LUMED may further provide specific privacy notices in line with specific activities and/or specific objectives.

Personal Data Collected

LUMED collects the following information, as may be applicable and necessary for its specific legitimate purposes

  • Personal details such as name, birth, gender, civil status and affiliations;
  • Contact information such as address, email, mobile and telephone numbers;
  • Medical information such as physical, psychiatric and psychological information;
  • Employment information such as government-issued numbers, position and functions;
  • Applicant information such as academic background, business information and previous employments; and
  • Academic information such as grades, course and academic standing.
  • Supplier and third-party provider information such as company profile, DTI/SEC registration, business permits, licenses, BIR registration, bank details and other information that it believes are relevant to meet the requirements of other government and non-government agencies and for any other legitimate purposes.

Method of Collection

  • LUMED collects Personal Data directly from its Data Subjects physically through printed forms, e-forms, e-mail, attachments, and other documents required by its medical and non-medical offices, or electronically through electronic systems, electronic platforms, e-forms, email, or electronic submission of information.

Timing of Collection

This is not a consent form but a notification on how LUMED processes Personal Data and Sensitive Personal Information of our Data Subjects. LUMED may further provide specific privacy notices in line with specific activities and/or specific objectives.

Legitimate Purpose of Data Collection

LUMED collects and processes Personal Data for the following purposes in accordance and in compliance with the Data Privacy Act of 2012:

  • Perform and conduct its functions and obligations as a healthcare provider;
  • Perform acts and decisions necessary for LUMED to manage and administer its affairs as a juridical entity with its own rights, interests and obligations;
  • LUMED may disclose, share and transmit personal data to hospital related entities and partners (including the members of medical staff, nurses, allied health care personnel and other medical staff);
  • Data may also be processed and transferred with other government/non-government agencies such as, but not limited to, Philippine Health Insurance Corporation, Department of Health, Health Insurance Providers or Health Maintenance Organizations, Department of Social Welfare and Development, National Archives, etc.

Storage, Location and Protection of Collected Data

Personal Data are stored in physical and electronic data processing systems managed by offices, sections, departments and units of LUMED. Physical records are generally stored in folders/envelopes/box files in drawers or shelves in a secure area. Electronic records are generally stored in our database maintained by our servers or in cloud storage in the possession or control of LUMED controlled.


Retention of Data Collected

LUMED retains data in accordance with its policies on retention observing laws and government rules and regulations set forth by different governing bodies such as the Department of Health, Bureau of Internal Revenue, Philippine Health Insurance Corporation, National Archives and other governing bodies.
In the absence of an applicable rule of retention, Personal Data shall be retained by LUMED group, office, or unit in accordance with locally and internationally accepted practices and standards.

Rights of our Data Subjects/Patients/Customers and Representatives

LUMED patients, service recipients, and their respective representatives and companions have the following rights with respect to their Personal Data:

  1. Right to be informed;
  2. Right to access;
  3. Right to data portability;
  4. Right to rectification and correction
  5. Right to file a complaint;
  6. Right to damages;
  7. Right to erasure and correction;
  8. Transmissibility of Rights
